3. If you have forms for us to complete or forms to return to us, please fax them to: 713-465-4522. 
4. If you have a form that needs to be completed by your medical team, and you DO NOT HAVE ACCESS TO A FAX MACHINE - please send it to:  clinicrotenberg@houstonspecialtyclinic.com for neurology patients.  This may cause an additional delay in completing the form.
