Practice Policies You may find this information helpful to understand some of the rules that make our practice run. Please note that these rules are subject to periodic updates.

Our office hours are Monday – Friday 8:30 – 5:00 PM. Phones are answered until 4:30 PM Monday – Friday.
In the event of an emergency, dial 911 and then contact us at our office. After hours, the office recording will give you the number to our page operator.
If you suspect your child has swallowed excessive /incorrect medicine or a poison, you may dial poison control at 1-800-POISON-1 or 1-800-222-1222.

Be on time and come early if your insurance or demographic information has changed
Children (and their grown-ups) can be filled with apprehension about coming to a specialist. Please plan ahead with distractions, snacks, diapers and CHARGING CORDS.
Paperwork - The first time you visit there are new patient forms you must fill out prior to the appointment. To make the process of filling out forms as painless as possible, we have put all our new patient forms on our website.
Please download and print the forms or request them via email so you can fill out the paperwork in the comfort of your own home.
If you do not have access to the internet, we can fax or mail the paperwork to you. If you are unable to fill out the new patient forms before you arrive at our office, please arrive at least 30 minutes prior to your appointment.
Cancellation We have many patients who need our help. Consequently, we require 24 hours notice if you are unable to make your appointment.
We take time to prepare for visits and procedures. "No-shows" and late cancellations use up resources that could be used for others.
We charge fees for no shows and short-notice cancellations.
For some labor intensive services, we may ask for a deposit to reschedule a procedure.
After 3 "no-show" appointments, your physician may release you from his care. If you are over 15 minutes late, we will do our best to work you in but we may have to reschedule your appointment.
Prescriptions If you need a refill on a prescription, call your pharmacy and request a refill for all non-controlled (non-"schedule 2") drugs. The Pharmacy will contact our office for approval for non-controlled drugs.
Controlled Prescriptions Please understand that these prescriptions are audited by the state and closely monitored. To meet our responsibilities, we employ staff and incur expenses beyond those for typical prescriptions. At the same time, we try to limit the burden on our families.
All controlled drug requests can be sent to your physicians Medical Assistant/Nurse by our HUB/Healow. There is an administrative fee for all controlled medications (triplicates). In addition, we need 2-3 business days to refill a controlled medication . Refills will be transcribed during office hours only.
We collect patient co-pays and/or deductibles prior to your seeing the physician. Please be prepared. We accept cash, check, MasterCard and Visa.
Insurance have a variety of ever-changing coverage rules, limits, exclusions. As a courtesy, we do our best to check your coverage in advance. Still, your insurance may not cover the full cost of your charges for service. Regardless of insurance, payment remains your personal responsibility and is due before you see the physician.
Forms and Letters It seems like there are more requests for forms every day. These take time to complete. And, as a specialty practice, almost all of our patients needs forms.
There is an administrative fee for forms and letters completed or certified by the physicians. The fee is collected prior to when the form or letter is released. Blank forms will not be accepted, parent’s portion must be filled out before being submitted to the physician. We require a 5-7 (3-5) business day turn around time for forms and for letters.
There are rare forms are so complex that they require a patient visit.
Dental, School/Camp Forms
In order for our doctors to fill out a physical, dental, school/camp form, your child must be up to date on his/her visits.
Mailed in, faxed in, emailed in: Forms do not need to be handed to our office in person. They can be mailed in, faxed in, or emailed to us.
Allow 1 week: Please allow our office at least 1 week (5-7 business days) to process and return your form.
Express process: Forms requiring same day express processing will be subject to an added fee since it disrupts clinical flow.
Medical records We charge a nominal fee for printing records. The fee is collected prior to receipt of records.
