Information collected from this website or provided on any form you have submitted through the website is used only in conjunction with an expressed interest by the user in obtaining additional information about our services.
It does not create any type of relationship between the individual providing the information and HSC or any of its healthcare providers. This information is not considered Protected Health Information (PHI) and will be used to contact you because you have requested that you be contacted.

When sending an e-mail, please put the subject of your message in the subject line so we can process it more efficiently. Also, be sure to include your full name and return phone number in the body of the message.
We also request that you acknowledge receipt of e-mail coming from this office by using the auto-reply feature. Please contact the office directly for emergencies, schedule 2 / triplicate medication refills, mental health issues.
All email communications relating to diagnosis and/or treatment will be filed in your medical record.
About security, This office is dedicated to keeping your medical record information confidential. Despite our best efforts, due to the nature of e-mail, unauthorized third parties may gain access to messages. When communicating from work, you should be aware that some companies consider e-mail corporate property and your messages may be monitored. Even when emailing from home, you may feel access to your email is not well controlled, so you should take that into consideration. In addition, you should be aware that HSC staff members and physicians would have access to this information.
I understand that this office will not be responsible for information loss or delay or breaches in confidentiality that are due to technical factors beyond this office’s control. I understand that the practice periodically sends out a newsletter, education information and notices regarding upcoming events and that I have the option to be removed from that mailing list.
